Schedule for Accepting Passport Applications:

APPOINTMENT NECESSARY: Please call 810-231-1000 Ext. 219 to schedule your appointment to apply for a passport.

Please note that our office accepts your completed passport application and forwards it to Passport Services for an issuance determination. Only Passport Services makes issuance determinations. Since intake of passport applications can take some time, our office has established the following schedule for the intake of applications in order to better serve our residents:

Forms are available in the Treasurer’s Office or can be downloaded from the National Passport Information Center’s website: http://travel.state.gov.

We take great photos! $5.00 for both Township and non-Township residents.

A U. S. citizen needs a passport to depart and/or enter the United States and most foreign countries.

REQUIREMENTS

Completed Application (DS-11)

You can obtain the Form DS-11 from Hamburg Township or by accessing the form at the U.S. State Department website.

Complete the application in black or blue ink only.

DO NOT sign the application until given oath by clerk.

Proof of Citizenship

* Certified copy of birth certificate issued by an official governmental agency. A certified copy will have a raised seal. (Hospital birth certificates are not acceptable.) (Additional documentation may be needed for persons that have been adopted or have had a legal name change or have a delayed birth record).

* Previously Issued Passport

*Original Naturalization Certificate

Proof of Identity

¨*Adults - Valid Drivers License, State issued Identification Card or previously issued Adult Passport.

¨*Persons ages 16 to 18 can apply without a parent but must present a valid Drivers License or State issued Identification Card.

¨*Persons under age 16 must be accompanied by BOTH parents listed on child’s birth certificate. BOTH parents must present valid proof of identity. If only one parent can accompany the minor, a notarized statement of consent for passport issuance from the absent parent must accompany the application.

Checks or Money Orders

* Payment to the "U. S. Dept. of State" must be made by personal check or money order. Payment cannot be made to the U.S. Dept. of State by cash or credit card.

*Payment to Hamburg Township for the execution fee and photos can be made by cash, check or money order. We do not accept credit cards. Payment to the township for multiple applications can be made with one check.

Allow 6-8 weeks for a regular application.

Allow two weeks for Expedited Service. (Expedited Service costs $60 more per application, payable to the U. S. Dept. of State. If you also desire overnight mailing service, the township charges an additional $20 for Standard Overnight or $22 for Priority Overnight).

You can apply in person at the Chicago Passport Agency located in the Kluczynski Federal Building, 230 S. Dearborn Street, Chicago, Illinois 60604. You must have an appointment. The phone number is 1-877-487-2778. The agency is open Monday – Friday from 9:00 a.m. – 4:00 p.m. Central Time. Customers may schedule an automated appointment 24 hours a day seven days a week by calling 312-341-6020. In addition to the normal forms, photos, identification and fees, you will need proof of immediate travel plans (airline tickets and itinerary) or a letter from your employer detailing your need for immediate business travel.

Or, there are passport expediting services that will hand carry your application for you – for a fee, of course! Your travel agent can probably recommend one of these agencies. You still have to apply at a Passport Acceptance Facility, such as ours; but instead of mailing your application it is forwarded to the private service, who makes an appointment, obtains the passport for you and gets it back to you either through your travel agent or by express mail.

 

If the following criteria apply, you may renew by mail: (Form DSP-82 You can obtain the Form DSP-82 from Hamburg Township or from the internet at: www.state.gov/m/a/dir/c5856.htm.). Renewal fee is $75.00.

  • Your previous passport must have been issued on or after your 16 th birthday.
  • Your passport must have been issued within the last 15 years.
  • If your name has been changed due to marriage or by court order, you must submit proof of the name change.
  • If a passport was issued as a child, that passport cannot be used to renew for an adult passport.
